ONLINE INFORMED CONSENT FOR MASSAGE

Before receiving massage therapy services, clients should understand the following information.

Massage therapy services are provided by Jason Butts, Licensed Massage Therapist (Texas License #MT148520) operating under Massage Establishment License #ME10518 in Sweetwater, Texas.

Nature of Massage Therapy

Massage therapy involves manual manipulation of soft tissues including muscles, fascia, and connective tissues.

Massage may be used for: Relaxation, Reduction of muscle tension, Improved circulation, General wellness support

Client Responsibility

Clients agree to: Provide accurate health information, Inform the therapist of any injuries or medical conditions

Communicate discomfort during the session, Update the therapist regarding changes in health status

Contraindications

Massage therapy may not be appropriate for certain medical conditions.

Clients should inform the therapist if they have: Recent injuries, Recent surgeries, Severe pain, Blood clotting disorders

Contagious skin conditions, Fever or illness, When appropriate, clients may be advised to obtain medical clearance before receiving massage therapy.

Scope of Practice

Massage therapy services are provided within the legal scope of practice defined by the Texas Department of Licensing and Regulation (TDLR). Massage therapy does not include: Medical diagnosis, Medical treatment, Chiropractic adjustments, Physical therapy services

Client Comfort

Clients may request adjustments at any time including: Pressure level, Positioning, Areas of focus, Termination of the session, Client comfort and safety are priorities during all sessions.
